One way we accomplish this is by equipping the adolescents and teens entrusted to our care with transferrable treatment tools so that they can continue their recovery at home. Throughout their time at Southstone, we encourage our guests to take the skills they develop during treatment and apply them after they leave our care. We also encourage continued recovery through a thorough discharge planning, or aftercare, process.
At Southstone, we value your child’s aftercare process as much as we value their residential treatment experience. We prepare each guest for a seamless transition by developing a discharge plan. Similar to their treatment plan, each adolescent’s discharge plan is created by their treatment team, starting at the beginning of their stay. Your child’s discharge plan is subject to change based on their changing needs throughout their time at Southstone.
Each adolescent’s discharge plan will include specific details on how best to continue their recovery after leaving our campus. Elements in your child’s discharge plan may include ongoing medication management services, in-home therapy, or traditional outpatient therapy. Southstone maintains reliable contacts throughout the area in order to provide our departing guests with the best step-down resources available. If you and your child have a preferred outpatient provider, Southstone will work with them to determine if they can offer the appropriate level of follow-up care for your adolescent. Southstone will remain in contact with each adolescent and their family for the following six months after they leave our campus, ensuring the stability of their discharge plan.
